About the Role

We are seeking a compassionate and skilled Counsellor or Social Worker to join our dedicated team. In this role, you will be responsible for providing trauma-informed evaluation and intervention to clients facing a broad spectrum of systemic barriers, including but not limited to homelessness, domestic and family violence, mental health challenges, and issues related to alcohol and other drugs (AOD). Your work will be critical in helping clients overcome these barriers and achieve positive, sustainable outcomes in their lives.

 

As a Chatr Clinician, you will employ a client-centered, strengths-based approach, ensuring that every individual’s inherent worth and capacity for growth are recognized and supported. You will work closely with clients to identify their goals, develop personalized intervention plans, and provide the necessary support to help them navigate complex social and personal challenges. This role requires a deep understanding of the factors that contribute to systemic disadvantage and a commitment to advocating for and empowering clients within an outcomes-focused Employment Service delivery framework.

Key duties

Client-Centered Interventions: Utilize a client-centered approach to engage with individuals, understand their unique needs, and develop tailored intervention strategies. Apply a strengths-based methodology to empower clients, helping them build on their existing capabilities and resources.

  • Comprehensive Case Management: Conduct thorough case management to plan, coordinate, and monitor the progress of clients. This includes setting achievable goals, tracking outcomes, and adjusting plans as needed to ensure that clients receive the most effective support. You will also be responsible for maintaining detailed records of all client interactions and progress.

  • Support & Advocacy: Provide robust support and advocacy for clients, facilitating their access to external services, medical professionals, and community agencies. This may involve coordinating multi-disciplinary teams, attending case conferences, and ensuring that clients receive holistic care. In-person support may be required to accompany clients to appointments or meetings with external service providers.

  • Assessment & Reporting: Conduct accurate and thorough assessments of clients' needs, documenting their progress and outcomes. You will prepare reports that meet the contractual requirements of our programs, ensuring that all necessary information is conveyed clearly and professionally to stakeholders.

  • Stakeholder Engagement: Actively engage with key stakeholders, including community organizations, government agencies, and healthcare providers, to develop and maintain strong, collaborative relationships. Your ability to foster these partnerships will be essential in driving successful client outcomes and ensuring that Chatr Allied Health continues to deliver high-quality services.

  • Technology & Systems Proficiency: Demonstrate a high level of proficiency in using technology and systems essential for case management and reporting. This includes maintaining accurate digital records, utilizing case management software, and ensuring compliance with data protection and privacy regulations.

    About You

  • Bachelor or equivalent Counselling,  Social Work, Behavioural Science or any other Allied Health degree

  • A satisfactory criminal record (case by case as we do support people with lived experience)

  • A valid Australian Drivers License and access to own vehicle

  • Excellent ability relationship building ability from clients to key stake holders

  • You will be self-motivated, with excellent time management skills and have a willingness to demonstrate initiative
